| Dentist identifies patient requiring a referral and discusses treatment options with patient. |
| Referring practice completes a referral form or writes a letter or telephones us with patient details |
| Patients consultation appointment made. |
| Pentangle sends our ‘Welcome Pack’ to patient with an appointment card, maps to the practice and details of the relevant dentist they will be seeing. |
| Patient has consultation – Current fee is £150 inclusive of periapical radiographs |
| Copies of Treatment Report and Treatment Plan with costs is sent to both patient and referring dentists in patient folder. |
| Copies of treatment undertaken by either referring dentist or Pentangle dentist will be communicated to each other at each stage of treatment. |
| At the end of treatment, patient is returned to referring practice. Pentangle Dental Transformations will write confirming completion of active treatment letter to the referring practice, and will give details of review appointments required. |
